Electronic Invoicing & Document Delivery

I confirm that I am authorized to make the election of electronic receipt of invoicing and policy documents on behalf of my organization. We agree to receive all Insurance Board documents electronically and will no longer be mailed hard copies of invoices and policy documents. Electronic receipt of invoices and policy documents will remain in effect unless Insurance Board is notified in writing. Documents will be sent from "notifications@origamirisk.com"
