Certificate Requests

Effective immediately, please follow the below process for ordering Certificates of Insurance:

Requesting a New Certificate of Insurance for the package, our core insurance program with Marsh & McLennan, please complete a COI request form (package, see below) and e-mail it to certificates@InsuranceBoard.org.

Certificates for the package, our core insurance program, can also be ordered online using the CSR24 Client Portal.

  • Only the insured participant and sub-agent will receive a copy of the MOI, although our system generates notices to mortgagees and loss payees as part of the cancellation process.  If you need to set-up a recurring certificate for each Loss Payee, please make sure to order a certificate.

Requesting an Auto ID Card or WC COI for our program with the Hylant Group, please complete a COI request form (Auto/WC, see below) and e-mail it to auto-wc@InsuranceBoard.org.
