To assist our member churches in the claims process, we help by:

  • Acting as a friendly professional liaison in the reporting of claims
  • Contracting a national Claims Service network to provide 24-hour claims processing
Claims Procedure: 
Please have the following information available for all claims:
  • Contract Numbers (see below for specific instructions depending on the type of claim) 
  • Your Church/Organization name, address, phone & UCCIB ID Number
  • Date, time, location and description of loss
  • Names, addresses and telephone numbers of relevant parties and witnesses
  • Police Report, if applicable
 


If you wish assistance reporting a claim you may contact the Insurance Board between 9AM and 5PM EST weekdays at 1.800.437.8830. Follow the instructions to access the Claims menu (by pressing “4”). And then f
ollow the Claims menu for instruction on reporting specific types of claims:

  • Press “1” for Property claims, reported to Constitution State Services (Travelers). Use the following Property Contract Number for Travelers: CFTCSSC-306T2149
  • Press “2” for Zurich claims, including Auto, Workers’ Compensation, and General Liability. You will need your Zurich policy number.
  • Press “3” for Richard Livingston, or to report Directors & Officers Liability, Sexual Misconduct Liability or Professional Liability claims.
  • Press “4” for Yvette Johnson, or additional assistance from a UCCIB representative.

Once your claim is reported, you will receive a phone confirmation within 24-hours of referral. A written confirmation will be mailed to you from the Claims Administrator.


REPORTING A 24-HOUR EMERGENCY PROPERTY CLAIM
To report after-hours emergency Property claims, phone Constitution State Services claim reporting (Travelers), our third-party claims administrator, at 1-866-385-0529. Use the UCCIB’s Property Contract Number for Travelers: CFTCSSC-306T2149.

REPORTING A GENERAL LIABILITY, AUTO OR WORKERS’ COMPENSATION CLAIM
To report a claim directly to Zurich North America, for General Liability, Auto, or Workers’ Compensation claims, dial the 24-hour claims reporting line at 1-800-987-3373. You will need your Zurich policy number.

    Additional Information Needed

  • Liability Claims
    • Name, address and phone number of claimant
    • Copy of lawsuit or other legal documents, if applicable
    • Please send copy of any lawsuit to the UCCIB
  • Automobile Claims
    • Vehicles involved - owner, ID #, year, make, model
    • Driver's address, license #, and phone #
    • Location of damaged vehicle (garage, etc.)
  • Workers Compensation Claims
    • Name & address of employee / volunteer
    • Social Security Number
    • Date of birth
    • Sex
    • Marital status
    • Number of dependents
    • Date of hire
    • Anticipated time off the job
    • Average weekly wage
    • Type of injury and affected body part
    • Full-time or part-time status
    • Employee or volunteer
    • Name and address of treating physician or hospital

The Claims Administrator will complete the required form, mail you a copy, and will also file the Employer's First Report of Injury with the state Workers Compensation bureau or board.


What to do When a Claim Occurs—
In the event of a loss, damage, or accident, you should do the following:

  • Notify the police if a law may have been broken.
  • Report the claim as soon as practical. (Refer to the Claim reporting procedures above.)
  • Provide a description of how, when, and where the loss or accident occurred.
  • Take reasonable steps to protect against further loss or damage.
  • Immediately send copies of any demands, notices, summonses or legal papers received in connection with the claim or lawsuit.
  • Authorize the release of relevant records and other information needed to process the claim.
  • Cooperate in the investigation, settlement, or defense of a claim or lawsuit.
  • Advise us of any suspicious circumstances about a claim.
  • Preserve evidence relating to an incident, e.g., potentially defective equipment.
You should not do the following:  
• Delay reporting while collecting information about an accident, injury or claim.
• Assume any obligation, make a payment, settlement, or incur expense, other than necessary first aid or emergency repairs, without the insurance carrier's consent, except at your own cost.
• Do anything after an incident occurs that would interfere with the right to recover from others.
• Provide statements, written or oral, except those required legally by proper authorities, to anyone other than an authorized representative of the United Church of Christ Insurance Board.
